The Support Coordinator (Case Manager) has a responsibility to follow this cycle (above) to ensure Support Services make sense and are pertinent to the individual client's success.
The cycle begins by Coordinating a level of Support Service Funding with Government Agencies, such as the Division of Services for People with Disabilities (DSPD), and the Department of Human Services (DHS). Since this funding is from State Tax dollars, as well as a Federal Medicaid match, it is important to implement them wisely and track them closely.
Once the funding is allocated, the Support Coordinator will assist in Assessing the needs and wants of the individual, and help design supports to address those wants and needs specifically. Goals and supports will be put into a Person-Centered Support Plan (PCSP), and data collection will be outlined to document progress and other achievements.
In addition, the Case Manager will assist in Linking appropriate and meaningful community resources to augment the established plan, whether these are simply for recreation, or needs such as adaptive equipment or specialized medical.
Ongoing Monitoring then swings into place. Regular contact is made each month, with follow ups as needed. This ensures that the goals, supports and services are working, and that they continue to make sense for the client's success. Adjustments can be made and implemented at any time to address concerns, issues, or life changes that may arise for the client.
The cycle repeats as the process for planning and evaluating the level of needed funding is renewed at least annually. This is done while focusing on ensuring the health, safety, independence, quality of life, and overall happiness of the individuals we serve.
